Records must be reconstructable
A useful cultivation record allows another qualified person to understand what happened, to what, where, when, by whom, and with what result. Context matters. “2.1” is not meaningful if the record does not show whether it is pH, EC, another measurement, which sample/location it belongs to, or when it was taken.
Scientific foundation — records are part of the measurement system
A measurement is not complete when the instrument displays a number. Its meaning depends on identity, units, time, location, instrument status, method and surrounding conditions. If any of those are missing, the operation may not be able to interpret or reproduce what happened.
Think of a valid measurement record as:
value + unit + object/sample identity + location + time + method/instrument context + operator/system identity
This is why documentation errors can create scientific errors. A correct number attached to the wrong plant, room or time can lead to a wrong conclusion just as easily as an inaccurate measurement.
WHO’s data-integrity framework uses the ALCOA+ concepts:
- attributable;
- legible;
- contemporaneous;
- original or true copy;
- accurate;
- complete;
- consistent;
- enduring;
- available.
The course uses these as transferable quality concepts, not pharmaceutical law.
Record facts when they occur
A value entered much later from memory is weaker evidence than a value captured at the time of observation. If delayed entry is unavoidable, follow the controlled method and preserve the true timing instead of making the entry appear contemporaneous.
Corrections preserve history
A correction should not silently erase the prior state. Paper or digital systems should preserve the original information and the fact of the correction according to the site procedure. Missing data remain missing until they can be legitimately resolved; do not invent them.
Worked case
A technician wrote an incorrect EC value on a paper form. The stronger correction preserves the original entry, records the corrected value using the approved method, and keeps the change attributable. Erasing the original or rewriting the sheet destroys history.
Guided practice — repair the record
Correct a mock cultivation record containing:
- missing units;
- ambiguous batch ID;
- an overwritten value;
- a retrospective entry presented as real-time;
- an unexplained blank;
- a copied value from a different zone.
For each issue, explain the data-integrity principle involved.
